29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a) Employees were exposed to the hazard of falling approximately 15 feet where fall protection was not provided at front elevation window openings on the second story of the building.     b) Employees were exposed to the hazard of falling approximately 15 feet where fall protection was not provided at a side elevation wall opening on the second story of the building.     c) Employees were exposed to the hazard of falling approximately 8 feet where fall protection was not provided at a side elevation wall opening along the stairway leading from the first level to the second level of the building.     d) Employees were exposed to the hazard of falling approximately 10 feet where fall protection was not provided around the foundation wall above the basement.     e) Employees were exposed to the hazard of falling approximately 10 feet where a floor hole cover was not secured or marked as a floor hole cover located above the basement.     No abatement certification or documentation required for the above violations; items a-e.     f) Employees were exposed to the hazard of falling approximately 22 feet while sheathing an 18/12 pitched roof of a residential structure.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation; item f (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a) Employees were exposed to the hazard of falling approximately 11 feet where a portable ladder did not extend above a landing surface along the interior front/side elevation of the second story of the building.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
