29 CFR 1910.36(b)(2): More than two exit routes were not available in a workplace when the number of employees, the size of the building, its occupancy, or the arrangement of the workplace is such that all employees would not be able to evacuate safely during an emergency:  On July 3, 2013 and at times prior thereto and thereafter, this facility was not provided with an adequete number of exit routes to accommodate the number of agents and other occupants in the process area.  Employees were exposed to possible fire or crush hazards during an emergency evacuation.

29 CFR 1910.36(c)(2): The street, walkway, refuge area, public way, or open space to which the exit discharge lead, was not large enough to accommodate the building occupants likely to use the exit route:  On July 3, 2013 and at times prior thereto and thereafter, the Sally Port, designated for this location as an emergency exit muster point for the process area, was inadequate for the number of agents and other occupants in the process area.  Employees were exposed to possible fire or crush hazards, during an emergency evacuation.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  In this federal building at Department of Homeland Security-Customs & Border Protection, on or about May 6, 2013, and at times prior thereto, the employer did not correctly record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012:   a. On or about May 6, 2013: An Agent working in the process area was exposed to dust and developed a bacterial infection in the lungs.  b. On or about May 19, 2013: An Agent working in the process area developed a lung infection and inner ear infection due to dust exposure.
