29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph: On or about July 10, 2013, at the above addressed jobsite, employees were accessing one side of the roof from a metal folding ladder on top of a scaffold at approximately 10 feet above ground, exposing them to a fall hazard.
