29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On or about July 15, 2025, at the above addressed jobsite, employees working below commercial roofing operations were not wearing a protective helmets, thereby exposing the employees to  struck-by hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about July 15, 2025, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about July 15, 2025, at the above addressed jobsite, employees engaged in commercial roofing operations were exposed to eye or face hazards from flying particles while operating  pneumatic nail guns without eye protection.  MK Construction Group, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1684483, citation number 1, item number 1 and was affirmed as a final order on September 5, 2023, with respect to a workplace located at 1212 Columbian Ave, Oak Park, Illinois, 60302.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about July 15, 2025, at the above addressed jobsite, employees performing commercial roofing operations, eleven (11') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.  MK Construction Group, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1780231, citation number 1, item number 1 and was affirmed as a final order on December 26, 2024, with respect to a workplace located at 399 Fountain Dr. Elgin, Illinois 60124.
