OSH ACT of 1970 Section (5)(a)(1): Each employer shall furnish each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees; and shall comply with occupational safety and health standards promulgated under this act.  Employees operating and working near the special purpose manlift in the east elevator were exposed to fall and struck by hazards for the following items:  a) The ground level for the manlift was not provided with a hoistway door at least 80 inches in height, exposing employees to struck by hazards.   b) The upper level for the manlift did not have the hoistway landing entrance door installed, exposing employees to fall hazards.    Among others, one feasible and acceptable method is to install hoistway doors at the groung level and upper landing as stated in American Society of Mechanical Engineers (ASME) A17.1-2010/CSA B44-10 (2010) as stated in paragraph  2.11.1.1 Hoistway landing entrances.  All elevator hoistway landing openings shall be provided with entrances that shall guard the full height and width of the openings.  Entances shall be at least 2,030mm (80 in.) in height and 800 mm (31.5 in.) in width.

29 CFR 1910.23(a)(2): Every ladderway floor opening or platform shall be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening:  Employees accessing the roof of the west elevator gallery were exposed to fall hazards of  7 feet, 5 inches from the ladderway opening to the concrete bin deck level below.  The ladderway opening did not have a swinging gate or offset.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:  Employees were exposed to fall hazards in that open-sided platforms were not guarded in accordane with the standard:  a) west elevator, a section of the open-sided floor measuring 4 feet 10 inchest, was 15 feet 2 iinches from ground floor to concrete floor of the boot pit.   b) west elevator, wall to floor distance of 30 inches, does not meet the requirement for a standard guardrail. Distance from top of wall to concrete below is 13 feet, 2 inches.
