29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment was used for the purpose for which it was intended:  Employees performing housekeeping and maintenance tasks on the motor level of the headhouse were exposed to fire and electrical shock hazards.  A 120 volt AC power strip mounted on the wall supplying power to the telecommunications equipment was not used as it was listed and labeled.

29 CFR 1910.305(a)(2)(ii): Temporary electrical power and lighting installations over 600 volts, nominal, were used at times other than during periods of tests, experiments, or emergencies:  Employees performing housekeeping and maintenance tasks were exposed to electrical shock and fire hazards. An extension cord was used to power a telecommunications repeater mounted on the roof.

29 CFR 1910.305(a)(2)(x): 29 CFR 1910.305(a)(2)(x):  Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  Employees performing housekeeping and maintenance tasks were exposed to electrical shock and fire hazards. An extension cord used to power a cable communications provider's repeater was run through a metal frame window.

29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    Employees performing housekeeping, maintenance, and truck loading tasks were exposed to fall hazards through ladder openings at the following locations:    a) Truck load out platform on the south side of the grain elevator.  The fall hazard measured 10 feet 7 inches.  b) Motor platform inside the head house.  The fall hazard measured 10 feet to the concrete floor below.  c) Distributor floor to the ladder platform.  The fall hazard measured  12 feet 6 inches.  d) Ladder platform to the bin deck level.  The fall hazard measured 9 feet 6 inches.  e) Catwalk to concrete elevators to the bin deck level.  The fall hazard measured 6 feet 4 inches.    The Pawnee County Cooperative Association was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(2), which was contained in OSHA inspection number 917414, citation number 01, item number 002 and was affirmed as a final order on 8/30/2013, with respect to a workplace located at 400 Ave. and US Hwy 56, Garfield, Kansas.
