29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Plant - On or about 07/10/13 employees performed lockout/tagout on equipment including (but not limited to) the truss presses (seven units), the ALS linear saw, The Auto Mill, the splicer, and the grinder. Workers were exposed to rotating parts, electrical energy, and compressed air. The energy control procedures did not clearly and specifically outline: A) A statement on the intended use of the procedure, B) The steps for shutting down, isolating, blocking, and securing machines to control hazardous energy, C) The steps for placement, removal, and transfer of lockout/tagout devices and the responsibility for them, D) The requirements for testing the machines to determine and verify the effectiveness of lockout/tagout devices and other energy control measures. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: a) Plant - On or about 07/10/13 employees performed lockout/tagout on equipment including (but not limited to) the Alpine Truss Machines (Presses) (7 units), the Alpine Splicer Machine (480 V), the ALS Linear Saw (230 V), and the Alpine Auto Mill. Workers were exposed to electrical energy, compressed air, and moving and rotating parts. The employer did not certify that the inspections of the individual lockout/tagout procedures were conducted and maintained up to date. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
