ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/16/13, on the jobsite at the Catholic Charities Office building located at 76 Humboldt Pkwy., Buffalo, NY: Employees engaged in roofing work on steep & flat areas of the roof ere exposed to falls of approx. 10 to 24 ft. in the different areas because no fall protection system was utilized. The roof pitch was estimated to be approx. 8 in 12 in the steep areas. Double rows of slide guards, had been installed as fall protection in some areas, but this does not meet the OSHA fall protection requirements of the standard. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a) On or about 07/16/13, on the jobsite at the Catholic Charities Office building located at 76 Humboldt Pkwy., Buffalo, NY: Employees engaged in roofing work were exposed to a potential fall of approx. 23 ft. to the ground when an extension ladder was used to access the "pic" of a ladder jack scaffold erected on the west side of the building. The siderails of the ladder only extended approx. 8 inches above "pic". b) On or about 07/16/13, on the jobsite at the Catholic Charities Office building located at 76 Humboldt Pkwy., Buffalo, NY: Employees engaged in roofing work were exposed to a potential fall of approx. 12 ft. to the ground when an extension ladder was used to access the lower roof on the front side of the building. The siderails of the ladder only extended approx. 18 inches above roof. NO ABATEMENT CERTIFICATION REQUIRED
