29 CFR 1926.106(d): At least one lifesaving skiff shall be immediately available at locations where employees are working over or adjacent to water. (a) Job Site - On or about July 18, 2013, employees were working over water while demolishing a highway bridge. A lifesaving skiff was not available for rescuing employees from the water.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite - On or about July 18, 2013, an employee was observed working at the edge of a bridge that was being demolished. The employee was not wearing a personal fall arrest system to prevent him from fall from the bridge and striking the ground or falling into the water.
