(a)  Bridge - On or about March 2, 2018 employees were exposed to an impalement hazard walking above rebar between 24 to 34 inches long. One employee stepped on top of uncapped rebar in order to climb the ledge of the bridge. The employer did not guard protruding reinforced steel.
