29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On July 23, 2013 an employee was not protected by hard hat when overhead hazards exisited, exposing the employee to a struck-by hazard.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.     On July 23, 2013 employees were not protected from a fall while working around a skylight, exposing employees to a fall hazard of more than six feet.
