On or about April 23, 2019, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.

On or about April 23, 2019, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:    a.  On or about April 23, 2019, at the above addressed jobsite, employees conducting roofing operations, three (3) stories from the ground, were not protected from falling.    Ageless and Comfort Roofing Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 989983, citation number 2 item number 1 and was affirmed as a final order November 19, 2014 with respect to a workplace located at 19515 Collin Drive, Frankfort, IL 60423.
