29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit" :   a)    Luke Air Force Base, Metal Technology Division: On July 16, 2013, and at times prior thereto, the metal shop supply room exit door leading to an outdoor location was not marked with a sign reading "Exit.  The lack of an appropriate exit door sign exposed workers to potential emergency evacuation hazards including safe egress in the event of a fire.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    a) Luke Air Force Base, Metal Technology Division shop: On July 16, 2013,  at times prior thereto, portable fire extinguishers in the welding booth and next to the main loading gate were blocked, not properly mounted, and not readily accessible exposing workers to serious fire injuries or death in the event of an emergency.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Luke Air Force Base, Metal Technology Division: On July 16, 2013, and at times prior thereto, the guard on the Herr-Voss Power Squaring Sheer had an inadequate barrier guard where the height of the weighted rollers guards was 1 1/4 inches and the distance of the point of operation hazard at 1 1/2 inches. Table 0-10 of OSHA 29 CFR 1910.217 states that for openings of 1 1/4 inches, the distance from the guard to the point of operation shall be 7 1/2 to 12 1/2 inches.   The guard is equipped with gravity based moveable rollers that lift easily to 1 1/14 inches.  They are not fixed and do not provide protection from hands entering past the roller guards, exposing workers to the point of operation.  No other method of machine guarding was provided.  Employees are exposed to potential hand and finger amputation hazards when feeding smaller size materials through the weighted rollers of the machine guard.

29 CFR 1910.265(c)(9)(i): Illumination was not provided and designed to supply adequate general and local lighting to rooms, buildings, and work areas during the time of use:   a)      Luke Air Force Base, Metal Technology Division Shop: On July 16, 2013, and at times prior thereto, the lamp on the ROMI to Tooling Machine was defective and not in working order.  The lamp provides local additional safety illumination to the work area and process.   Employees are exposed to amputation and other point of operation hazards due to a lack of adequate local lighting during the use of the retooling machine.

29 CFR 1910.1200(g)(8):     The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:    a)    Luke Air Force Base, Metal Technology Division: On July 16, 2013, and at times prior thereto, the employer could not produce a material safety data sheet for QuinSyn-Plus compressor lubricant.  The use or accidental spill and clean-up of the material exposes workers to irritation of mucous membranes and the upper respiratory tract.
