29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving: a) Luke Air Force Base, Utilities and Plumbing Operations Division workshop: On or about July 18, 2013, and at times prior thereto, the employer did not securely anchor the RYOBI 8 inch dual standing bench grinders to prevent walking or moving of equipment while in operation. Employees were exposed to serious injuries due to potential amputation, cuts, and crushing injuries due to the instability of the machinery.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: (a) Luke Air Force Base, Utilities and Plumbing Division workshop: On July 18, 2013, and at times prior thereto, both tool rests on the RYOBI 8" dual grinder had 1/2 inch openings which could result in work being jammed causing wheel breakage and serious injuries to workers operating the equipment.

Employees are exposed to electrical and mechanical hazards during emergencies that may require the shut off of circuits and/or equipment.
