Sufficient access and working space was not provided and maintained about the electrical switches to permit ready and safe operation and maintenance of such equipment and could result in serious electrical injuries.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective. a) Luke Air Force Base, LRS - AF Warehouse: On July 17, 2013, and at times prior thereto, the electrical wall outlet opposite the main electrical panels in the warehouse building had a false ground. Employees were exposed to electrical hazards and injuries when using the wall plug to power equipment and tools.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by 1910.178(l) of this chapter: The powered industrial truck operator's performance was not evaluated at least once every three years. a) Luke Air Force Base, LRS-AF Warehouse: On or about July 17, 2013, industrial truck operators did not received an evaluation of operator's performance every three years. Powered industrial truck operators were only trained initially when employed at this facility. Workers are exposed to serious injuries or death from being struck-by or crushed by forklifts at the warehouse.
