29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Luke Air Force Base, Civil Engineering Compound/ HVAV Unit, second flloor of building: On July 18, 2013, and at times thereto, an electrical boxe was not securely closed nor fastened by a suitable cover and thus exposed employees to electrocution hazards through accidental contact. Employees were exposed to serious electrical hazards from accidental contact with exposed wiring.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective. a) Luke Air Force Base, Civil Engineering Compound/ HVAV Unit: On July 18, 2013, and at times prior thereto, the 115V Plug on the HVAC Work Station was found to have a faulty ground exposing workers to serious electrical injuries.

Employees were exposed to electrical and mechanical hazards during emergencies requiring the shut off of circuits and/or equipment.
