29 CFR 1910.95(g)(8)(ii): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure that the following steps are taken when a standard threshold shift occurs:    (a) On or about 08/05/2013, in the hot metal area, for the employees that received a standard threshold shift.  The employer did not refer the employees for a clinical audiological evaluation or an otological examination, as appropriate.  Only a physician can make the determination that a standard threshold shift is not work-related.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a) On or about 08/05/2013, in the hot metal area, for the employees working with glass who wear prescription lenses in an area where safety glasses are required.  The employer provided slip-on side shields not meeting ANSI Z87.1-1989 to use on prescription lenses.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:  (a) On or about 08/05/2013, in the hot metal area, for the employees working with glass who wear prescription lenses in an area where safety glasses are required.  The employer provided slip-on side shields not meeting ANSI Z87.1-1989 to use on prescription lenses.
