29 CFR 1926.502(d)(15): Anchorages used for attachment for personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee: (a) Rafael Ramos Roofing at 1500 Hollyberry St., Berthoud, CO 80513: On and before July 3, 2013, the employer did not ensure that each employee was protected by the use of a complete fall protection system, in that employees laying asphalt shingles on a new residential home were attached to job made roof anchors not rated for the conditions. Condition exposed employees to a fall hazard of approximately 12.7 feet.

29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that employees can neither free fall more than 6 feet, nor contact any lower level: (a)Rafael Ramos Roofing at 1500 Hollyberry St., Berthoud, CO 80513: On and before July 3, 2013, the employer did not ensure that the personal fall arrest systems used by two employees limited the fall distance to 6 feet, nor prevented contact with the lower level. Two employees engaged in roofing activities were wearing fall harnesses attached to anchored vertical lifelines; however, their rope grabs were improperly adjusted so that the fall protection systems did not provide fall protection. Conditions resulted in employees exposed to fall hazards of greater than 6 feet, and with high potential of contacting the ground below at approximately 12.7 feet below.
