OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    On or about July 29th 2013, and at times prior,  Pioneer Motor of Trinidad Inc. employees were exposed to crushing and struck-by injuries in that:        a) The employer had not trained employees on the safe use and operation of the lifts using the manufacturer's provided instructions.     b) At least one out of nine lifts at this location did not have posted information related to the load capacity. 29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide:     a) On or about July 29th 2013, and at times prior, employees were exposed to struck-by falling object(s) hazards. There were at least three holes on the concrete slab of the 2nd floor body shop. The holes were the full thickness of the concrete slab, and about 4" in diameter. Technicians working on the first floor were exposed to falling object(s) hazards. An electrical outlet powering a wall mounted fluorescent light located on the east wall of the west shop had cracks on the cover.
