29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a- 7950 Ventnor Avenue, Margate NJ: An employee working beneath an employee performing EFIS installation and other related activities from the second buck of a prefabricated, tubular, welded framed scaffold was exposed to possible overhead struck by hazards when they were not supplied with head protection.  Condition observed on or about 07/15/13.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a- 7950 Ventnor Avenue, Margate NJ: On or about 07/15/13, a company employee performing EFIS installation and other related activities from a prefabricated, tubular, welded framed scaffold's second buck was exposed to falls of approximately 13 feet 7 inches to the ground surface below when the scaffold was not provided with mid/top rails and the scaffold's walking/working surface contained 3 planks of which the scaffold was designed for 6 planks.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a- 7950 Ventnor Avenue, Margate NJ: On or about 07/15/13, a company employee performing EFIS installation and other related activities from a prefabricated, tubular, welded framed scaffold's second buck was exposed to falls of approximately 13 feet 7 inches to the ground surface below when the scaffold's walking/working surface was not fully planked.  The scafffold's walking/working surface contained three planks but was designed by its manufacturer to hold six.
