29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems: a) On or about 08-08-2013 at the job site, where the roofs (4/12 slope) of apartment buildings, were being stripped and re-shingled, the use of fall protection was not enforced, exposing the employees to a 18 ft. fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 08-08-2013 at building #15, the 24 ft. aluminum ladder used to access the roof was only extended one rung higher than the roof surface exposing employees to an 18 ft. fall hazard.
