29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: At this location: The employer did not establish a lockout-tagout program for employees engaged in the servicing, maintenance or repair of equipment to prevent the sudden release of stored energy. Employees engaged in workplace activities without access to written lockout-tagout program were exposed to struck-by hazards while servicing, maintaining, or repairing equipment; such as but not limited to, mechanical/electrical concrete batch plant equipment, conveyor belt systems, concrete transporter or other worksite vehicles.

29 CFR 1910.147(c)(4): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At this location: The employer did not develop, document, and utilize procedures for the control of potentially hazardous energy while employees were engaged in washout activities of the Tuckerbilt T-630 Concrete Transporter device. Employee's not provided access to the Manufacturer's Cleaning Guidelines while engaged in vehicle auger washout tasks, were exposed to struck-by hazards from sudden contact with the vehicle rotating auger device.
