29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: HEI manufacture yard: On or about May 16, 2017, Four Hamilton Tensioning Jacks were missing a handles used to guide the jack into position at the point of operation. Operators had to grab and position the jack from the frame by hand exposing them to struck by and caught-between hazards.

29 CFR 1910.179(h)(4): Hooks did not meet the manufacturer's recommendations: On or about May 16, 2017, HEI beam molding yard; employees operating the 1 ton Jet hoist, used in pre-stressing cable operations was not being used in accordance with manufacture's specifications and limitations applicable to the operation, in that the latch for the hook had been removed. This condition exposed the employees to struck-by hazards.

This condition exposed employees to electrical shock hazards. a) Four extension cords used to power the internal concrete vibrators. b) Plug connecting the Tensioning (pre-stress) Jack control panel to power.
