The employer did not have machine specific written procedures for locking out/tagging out the multi-energy machines to safe guard employees performing services and/or maintenance on the machines, on or about August 8, 2013. a. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a. 5380-B Rafe Banks Drive, Flowery Branch, GA (Machine Shop Area) - Employees were exposed to fire hazard. The oxygen and acetylene cylinders used for welding repair work were stored together without a noncombustible barrier between them, on or about August 8, 2013.
