The employer did not institute a training program and ensure employee participation in the program:    a) Etori USA, Inc., Flowery Branch, GA - In the bend shop, an employee operating the drill press and grinding metal tubes was exposed to a projected noise level of 90.2 dBA, which exceeded the action level of 85 dBA. The exposure was derived from a sample taken over 362 minutes which represents the employee(s) 8-hour workday. Noise training was not conducted, on or about 6/5/15.    b) Etori USA, Inc., Flowery Branch, GA - In the bend shop, an employee punching holes on the punch press and cutting metal tubes was exposed to a projected noise level of 88.7 dBA, which exceeded the action level of 85 dBA. The exposure was derived from a sample taken over 366 minutes which represents the employee(s) 8-hour workday. Noise training was not conducted, on or about 6/5/15.    c) Etori USA, Inc., Flowery Branch, GA - In the bend shop, an employee cutting metal tubes was exposed to a projected noise level of 88.5 dBA, which exceeded the action level of 85 dBA. The exposure was derived from a sample taken over 362 minutes which represents the employee(s) 8-hour workday. Noise training was not conducted, on or about 6/5/15.    d) Etori USA, Inc., Flowery Branch, GA - In the weld shop, a welder operator I was exposed to a projected noise level of 87.3 dBA while welding and grinding. The exposure was derived from a sample taken over 346 minutes which represents the employee(s) 8-hour workday. Noise training was not conducted, on or about 6/5/15.    e) Etori USA, Inc., Flowery Branch, GA - In the weld shop, a welder operator III was exposed to a projected noise level of 86.8 dBA while welding and grinding. The exposure was derived from a sample taken over 382 minutes which represents the employee(s) 8-hour workday. Noise training was not conducted, on or about 6/5/15.
