29 CFR 1910.37(b)(4): When the direction of travel to the exit or exit discharge was not immediately apparent, signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge:     a.  5380-B Rafe Banks Drive, Flowery Branch, GA (Throughout Shop) - Employees were exposed to entrapment hazard. There were several designated, marked, and identified emergency exit discharge located throughout the facility which were hidden by large machineries equipment and storage materials, etc.  There were no directional exit markings indicating the way to travel, on or about 06/04/15.

Employees (material handling) constructed an attachment (man-lift basket) to go on the forks of sit-down forklifts. The lift basket is used to lift employees to retrieve items/parts from the upper storage racks. The man lift attachment was not approved by the forklift manufacturer and/or a qualified register engineer, on or about 06/04/15.

Employees operates the kosoku metal chop saws to cut metal/pipes to various sizes.  The saws were not adequately guarded. There was a section of the hood guard missing, covering portion of the rotating blade, on or about 06/04/15.   b. 5380-B Rafe Banks Drive, Flowery Branch, GA (Machinists Area) - Employees were exposed to struck-by and caught-between  hazard.  Employees operates the drills to punch/ream holes in the metal without having the guard in place, on or about 06/04/15.       a.  Millport drill         b.  Yamaschi drill  c. 5380-B Rafe Banks Drive, Flowery Branch, GA (Bender Area)  Employees were exposed to caught-between hazard.  Employees walk and work in close proximity to the bending machine while its in the process of bending the metal/steel part.  There is no machine guarding and/or barrier to prevent employees from coming in contact to the moving part if accidently slip and/or trip, on or about 06/04/15.
