29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substanially equivalent to the wall of the equipment: (a) Radio Shop, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491: On or about 1 August 2013, and at times prior thereto, the GENERAC EZ Switch panel cabinet mounted on the east wall had a one inch by two inch opening on the northside of the box which did not afford protection from electrical hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: (a) Radio Shop, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491: On or about 1 August 2013, and at times prior thereto, the legend for disconnect switches within the GENERAC EZ Switch panel box were mislabeled as to location and purpose for switches 9 and 11 (60 amp switches for Eastside HVAC Unit) as switch number 7 (20 amp switch for "lights other room"), exposing employees to hazards.
