29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 8/06/13 in the bedroom of apartment 105 in the basement of building under renovation at 786 Kenmore Ave., Buffalo, NY: A DeWalt Model DW124, 1/2" Right Angle Drill was used to drill holes in drywall. The drill was connected to 120-volt ac power via a series of two extension cords. The plug on the cord of the drill lacked a grounding prong. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) On or about 8/06/13 in the bedroom of apartment 105 in the basement of building under renovation at 786 Kenmore Ave., Buffalo, NY: A DeWalt Model DW124, 1/2" Right Angle Drill was used to drill holes in drywall. The drill was connected to 120-volt ac power via a series of two extension cords. The outer sheath of the second extension cord was frayed at the female end, exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
