29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. On or about August 13, 2013, an employee using a Hitachi NR83A2 pnuematic air nail gun did not have safety glasses while performing roofing operations, exposing himself to being hit by nails or flying debris.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed    a. On or about August 13, 2013, employees, accessing the roof on the east side of the house, used the top section of a 24 foot Werner extension ladder improperly.  The extension ladder was separated into two pieces and the top section was used independently from the base section as a free standing ladder, exposing the employees to an 8 feet 9-inch fall hazard.

The final order date was June 28, 2012.      Carlson Enterprises, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 446956, citation 01, item number 001, issued on November 7, 2012, with respect to a workplace at 181 Nadia Michelle Drive, Jacksonville, Florida 32225. The final order date was November 20, 2012.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.     a. On or about August 13, 2013, on a 6:12 pitch roof, five employees performing roofing operations were not protected from an 8 feet 9-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternate fall protection.
