29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on April 14, 2015, when employees installing roofing materials on the 4:12 pitch roof at 13033 Vilburnum Drive, Jacksonville, Florida, 32256 were exposed to an 8 foot fall hazard from the roof eaves in that they were not protected by a fall protection system.    Carlson Enterprises, LLC previously committed a Repeat Violation of this standard on December 15, 2014, when employees installing roofing materials on the 6:12 pitch roof at 9902 Blakeford Mill Road, Jacksonville, Florida, 32256, which became February 23, 2015.    Carlson Enterprises, LLC. previously committed a Repeat violation of this standard on August 13, 2013, when employees were working on a residential house at 2588 Spring Meadows Drive, Middleburg, Florida, 32050, which became a Final Order on September 16, 2013. On or about April 14, 2015, at the jobsite, employees were not wearing appropriate foot protection, exposing themselves to a foot injury.
