29 CFR 1910.212(a): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Mail Prep Area: The employer did not ensure that the machine operator and employees were protected from the rotating parts of an Phillipsburg Mail Star 400 Mail Inserter Machine at the point of operations. Employees were exposed to caught in hazards between the belt of the pully.

29 CFR 1910.303(g)(1)(ii): The required working space about electrical equipment rated 600 volts, nominal, or less to ground, was in a passageway or general open space and was not suitably guarded when normally enclosed live parts were exposed for inspection or servicing:    West corner of building in the Warehouse: The employer did not ensure that maintenance employees had sufficient working space (3-feet from the front and sides) provided for electrical panel rated at 600 volts or less. An electrical panel rated at 110/220 volt (controlling the lights in the warehouse area) was blocked by a table, chair and a portable hydraulic jack stand.

The panel had a missing breaker exposing employees to arc flash and arc blast.
