29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) At 672 Concord Rd SE, Smyrna, GA- Employees were exposed to electric shock hazards while altering a three outlet extension cord and connecting to the energized circuits located in a three-phase, 220 volt service disconnect. The employer failed to de-energize circuits before making these connections.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets: a) Roof of building at 672 Concord Rd SE, Smyrna, GA- The employer exposed employees to fall hazards of approximately 10 feet 8 inches while installing felt and shingles on a new roof without fall protection. K & C Roofing, Inc., was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13) and 29 CFR 1926.501 (b) (1) which were contained in OSHA Inspection Number 312775125, Citation #1, Item #1a and Citation #1, Item 1b, issued on 03/16/2010 with respect to a work place located at 2151 Cumberland Parkway, Atlanta, GA 30339 and became a final order on 04/09/2010.
