(b) On or about 01/19/2016 - at Building 5, 1 Stone Crest Dr., Birmingham, AL 35242, a 28-foot aluminum extension ladder used to access the roof was not extended at least 3-feet above the upper landing surface, was not secured to a rigid support, and/or a grasping device was not provided.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) On 01/19/2016 - at Building 5, 1 Stone Crest Dr., Birmingham, AL 35242, eye protection was not used when employees were operating a Hitachi NV 45AB2 Nailer and Hitachi Kiko EC 2510E Air Compressor to nail shingles, exposing employees to flying particles.  K&C Roofing, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1032185, citation number 2, item number 1 and was affirmed as a final order on 05/21/2015 with respect to a workplace located at 3737 St. John Bluff Rd S, Jacksonville, FL 32224.  K&C Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 931543, citation number 1, item number 1 and was affirmed as a final order on 11/22/2013 with respect to a workplace located at 672 Concord Rd SE, Smyrna, GA 30082.
