29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.  On or about February 3, 2015, at the west side of building No.3, two employees were exposed to the hazard of being struck in the head by falling objects in that they were not wearing hard hats while installing weather proofing paper on the first level canopy.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On the west side of building No.3, two employees using air nail guns to install roof shingles are not wearing eye protection, exposing themselves to being struck by flying debris or nails.  The K&C Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 931543, citation number 1, item number 1 and was affirmed as a final order on 11/22/2013, with respect to a workplace located at 672 Concord Rd SE, Smyrna, GA 30082.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about February 3, 2015, at the west side of building No.3, four employees installing weather proofing material and roof shingles on a 5:12 pitch residential roof were not protected from a 17ft-10in fall hazard by the use of a fall protection system.    b. On or about February 3, 2015, at the west side of building No.3, two employees installing weather proofing material on a 5:12 pitch canopy roof were not protected from a 8ft-6in fall hazard by the use of a fall protection system. c. On or about February 3, 2015, at the east side of building No.4, two employees removing old roof shingles on a 5:12 pitch residential roof were not protected from a 17ft-10in fall hazard by the use of a fall protection system.  The K&C Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 931543, citation number 2, item number 1 and was affirmed as a final order on 11/22/2013, with respect to a workplace located at 672 Concord Rd SE, Smyrna, GA 30082. K & C Roofing, Inc., was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13) and 29 CFR 1926.501(b)(l) which were contained in OSHA Inspection Number 312775125, Citation number 1, Items number la and 1b, issued on 03/16/2010 with respect to a work place located at 2151 Cumberland Parkway, Atlanta, GA 30339 and became a final order on 04/09/2010.
