29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) At the roof of the house located at Lot # 3, 3550 S Sherwood Rd, Smyrna, GA: Employee using Pneumatic Hitachi Nail Gun, Model: 83A2 (S1) was not protected from eye injuries while installing 1/2" OSB Plywood on the roof. b) Front side upper level window at Lot # 3, 3550 S Sherwood Rd, Smyrna, GA-Employee without eye protection was exposed to eye injuries while talking to an employee operating a Pneumatic Hitachi Nail Gun, Model: 83A2 (S1) while installing 1/2" OSB Plywood on the roof. Ramiro Perez dba GIOMW Construction,LLC, was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.102(a)(1) which was contained in OSHA Inspection Number 544898, Citation #1, Item #1, issued on 09/10/2012 with respect to a work place located at 736 Park Manor, Smyrna, GA 30080 and became a final order on 10/05/2012.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) Lot # 3, 3550 S Sherwood Rd, Smyrna, GA 30082: An employee installing 1/2" OSB plywood on a 12/12 pitched roof with an eave height of approximately 18 feet was not provided with a fall protection system. b) Front side upper level window at Lot # 3, 3550 S Sherwood Rd, Smyrna, GA-Employee without fall protection was exposed to fall hazards of approximately 13 feet above ground level while talking from a window opening to an employee operating a Pneumatic Hitachi Nail Gun, Model: 83A2 (S1) while installing 1/2" OSB Plywood on the roof. Ramiro Perez dba GIOMW Construction, LLC, was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 544898, Citation #1, Item #2, issued on 09/10/2012 with respect to a work place located at 736 Park Manor, Smyrna, GA 30080 and became a final order on 10/05/2012.
