29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.:    On or about August 15, 2013, and times prior to, at 4747 North State Highway 151 in Blytheville, Arkansas, in the Foundry, the ductile inoculation pit on the pour-off deck was not guarded.  This condition exposed employees to falls and severe burns.

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board:  On or about August 15, 2013, and times prior to, at 4747 North State Highway 151 in Blytheville, Arkansas, in the Foundry,  (a)  the furnace slag pit was not guarded to protect employees traveling back and forth on the adjacent walkway to retrieve ladles.  (b)  employees were not protected from falling from the metal catwalks/platforms above the slag pit.  These conditions exposed employees to falls from elevation and severe burns.

(G)    Carlton drill, #132073, located in the Machine Shop.  (H)    three Wilton VSG drills, located in the Machine Shop.  (I)     Richmond BFV drill, #13629, located in the Assembly area.    These conditions exposed employees to punctures and contact with rotating parts.
