29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: AIC International Inc.- Andersen AFB Clear Water Rinse Facility site - On or about August 16, 2013 and at times prior thereto, the walls of the excavation were not sloped in accordance with the Engineer's report nor was the excavation shored to prevent loose rock or soil that could pose a hazard by falling or rolling from the face of the excavation, or prevent the wall from collapsing, onto employee's working and walking inside the excavation and using the access ramp.

29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities were not in near proximity to the worksite and was not equipped as to enable employees to remove such substances: AIC International Inc.- Andersen AFB Clear Water Rinse Facility site - On or about August 16, 2013 and at times prior thereto, wash facilities for employees engaged in placing concrete in forms were not available.
