29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: This violation occurred on August 22, 2013, at a workplace located at 135 Rolling Fork, Irving, TX 75063; where at least two (2) employees, who were working at various elevations under construction activities, were not protected from head injuries by wearing hard hats. New Horizon Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a) which was contained in OSHA inspection number 312122021 citation number 1, item number 1 and was affirmed as a final order on 09/15/2009, with respect to a workplace located at 7322 Comal, Irving, TX 75063; and which was contained in OSHA inspection number 548978 citation number 2, item number 1 and was affirmed as a final order on 11/19/2012, with respect to a workplace located at 208 Rolling Fork Bend, Irving, TX 75039.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: This violation occurred on August 22, 2013, at a workplace located at 135 Rolling Fork, Irving, TX 75063, where at least one (1) employee, who was working from job-made scaffold platform 10-inches wide, was exposed to the hazard of falling to the ground below. New Horizon Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(2) which was contained in OSHA inspection number 312122021 citation number 1, item number 2a and was affirmed as a final order on 09/15/2009, with respect to a workplace located at 7322 Comal, Irving, TX 75063; and which was contained in OSHA inspection number 548978 citation number 2, item number 2a and was affirmed as a final order on 11/19/2012, with respect to a workplace located at 208 Rolling Fork Bend, Irving, TX 75039.
