29 CFR 1926.20(b)(2): The employer did not initiated and maintain programs that provided for frequent and regular inspections of the job sites, materials, and equipment by competent persons designated by the employers:  At the project on or about August 26, 2013, regular inspection of the job site were not conducted or the person that performed the inspection was not competent as there were several electrical hazards onsite, including no grounding pin on extension cords, handy boxes laying on the floor, extension cords spliced together, and extension cords outer insulation not secured to a connector to an electrical box.

29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that was reasonably accessible in terms of time and distance to the worksite, which was available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not available at the worksite to render first aid:  At the project on or about August 26, 2013, employees stated that no one onsite was trained in first aid. No one could produce any document that they were trained in first aid. The nearest medical treatment was more than four minutes away. The hospital was at least 20 minutes away. Employees used power drills and saws onsite.
