29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: A. Graduate Roofing Company, LLC worksite located at 2340 Parliment, Toledo, Ohio: On or about August 26, 2013, the employer did not ensure employees were protected from falls while installing weatherproofing on the south side of the single story residential structure. Employees were exposed to a potential fall distance of approximately 7.00 feet to the lowest level from the 3.5/12 pitched roof. Gradute Roofing Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 279562, citation number 2, item number 1 and was affirmed as a final order on May 4, 2012 with respect to a workplace located at Rossford, Ohio.
