29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Graduate Roofing Co. LLC worksite located 7337 W. Bancroft, Toledo Ohio 43615: On or about July 18, 2019, the employer did not ensure one employee was protected from head injuries while working below employees installing a trash chute from above. Due to this condition, the employee was exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: Graduate Roofing Co. LLC worksite located 7337 W. Bancroft, Toledo Ohio 43615: On or about July 18, 2019, the employer failed to ensure a warning line system was installed at a distance equal to or greater than 6 ft. from the roof's edge. Employees performing roofing activities at a location where the line was maintained at 47 inches from the edge, were exposed to falls of up to 17 ft. to the ground below.
