29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.  a. Table, Punch Press Area, Near the Amada PEGA -  The employer did not directly connect a Waber-by-Tripp Lite 6SP Power Strip or relocatable power tap into a permanent receptacle.  An operating TPI Industrial Pedestal Fan was plugged into the power strip.  This condition exposed the employees to an electrical shock. Observed On or About August 26, 2013

29 CFR 1910.303(b)(8)(i): Electric equipment was not firmly secured to the surface on which it is mounted.    a. Table, Punch Press Area, Near the Amada PEGA - The employer did not securely fastened a metal receptacle box to the surface.  An orange color flexible cord was wired into a metal receptacle box with pre-punched knockouts and this assembly was being used as a portable electrical extension cord to supply electrical power to a Waber-by-Tripp Lite 6SP Power Strip.  An operating TPI Industrial Pedestal Fan was plugged into the power strip. This condition exposed the employees to an electrical shock.  Observed On or About August 26, 2013

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective.  a. Table, Punch Press Area, Near the Amada PEGA -   An orange color flexible cord was wired into a receptable box and the ground wire was not attached to box.  This assembly was being used as a portable electrical extension cord to supply electrical power to a Waber-by-Tripp Lite 6SP Power Strip.  This condition exposed the employees to an electrical shock. 29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter shall be effectively closed.   a. Table, Punch Press Area, Near the Amada PEGA -  Conductors entering an opening in a metal receptacle box were not protected from damage if they rub against the opening.   An orange color flexible cord was wired into a metal receptacle box with pre-punched knockouts and the strain relief had been pulled out of the box exposing the conductors to unprotected metal edge of the box.  This condition exposed the employees to an electrical shock. Observed On or About August 26, 2013

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  a. Painting Department, Wet Paint Spray Booth - The employer did not ensure that employees were fit tested for their respirators prior to initial use of their respirators.  Employees wore 3M 6000 Series air-purifying half-mask respirators equipped with 3M Particulate Filters 5N11, N95 plus 3M 6006 Multi Acid Gas Organic Vapor Cartridges while spraying water-based paint onto parts inside an open face industrial paint spray booth.  The employer required these employees to wear these respirators.  Observed On or About August 26, 2013
