29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk were not guarded by either standard railing with standard toeboard on all exposed sides, or by a floor hole cover of standard strength and construction: On or about August 30, 2013, the expanded metal flooring in several locations in the processing area(s) had numerous open floor holes present that could lead to tripping or falling hazard(s) such as, but not limited to: a. At the load line, between blend tanks 101 and 102 a floor opening of approximately 6 inches by 8 inches was present; b. At the #6 blend tank a floor opening of approximately 4 inches and 6 inches was present; c. At the #7 blend tank a floor opening of approximately 4 inches by 6 inches was present; d. On the first floor of the reactor building by column B4 a floor opening of approximately 4 inches by 6 inches was present; e. On the second floor in the finishing area by the Eimco #1 discharge pipe a floor opening of approximately 9 inches by 9 inches was present.
