29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):        a) Employees were exposed to the hazard of falling approximately 12 feet where fall protection was not provided at and around floor holes on the second story of the building.     b) Employees were exposed to the hazard of falling approximately 12 feet where fall protection was not provided at and around window and wall openings on the second story of the building.     No abatement certification or documentation required for the above violations.   c) Employees were exposed to the hazard of falling approximately 12 and 22 feet while performing carpentry work from the top level of the building without the benefit of fall protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation; item f (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      a) The employer did not provide training to enable employees to recognize the hazards of falling and the procedures to be followed to eliminate or control such hazards at the worksite.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIV ACTION WORKSHEET).
