29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation. a) On February 11, 2026, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nailers and saws without eye protection. No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) On February 11, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of approximately 10 feet 6 inches without the use of fall protection. Employees were exposed to interior falls from the top of trusses to the interior floor. b) On February 11, 2026, at the above addressed jobsite, an employee engaged in framing activities was exposed to falls of approximately 13 feet 6 inches without the use of fall protection. Employees were exposed to interior falls from the top of trusses to the interior floor. c) On February 11, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of approximately 23 feet without the use of fall protection. Employees were exposed to exterior falls from the top plate of the second story walls to the exterior ground on the of the house being constructed. No abatement certification or documentation is required for this item.
