No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: a) On or about February 18, 2016, employees were exposed to fall hazards of approximately 15.2 feet while performing carpentry work on a second story deck. b) On or about February 18, 2016, employees were exposed to fall hazards of approximately 17 feet while performing carpentry work around unguarded window openings. Leinster Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 937135, citation 1, item 1, and was affirmed final order on or about September 17, 2013, with respect to a workplace located at 4832 N. Clark St., Chicago, IL, 60607. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Clark St., Chicago, IL, 60607. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
