Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish to each of his employees employment and a place of employment which were free from the recognized hazards of being struck-by and/or crushed-by machinery and/or equipment which was causing or likely to cause death or serious physical harm to employees: This violation most recently occurred on or about September 4, 2013, where employees operated overhead cranes that had broken or missing safety latches at the throat of the hooks, exposed to struck-by and/or crushed-by hazards, in the following instances: a. An overhead crane at the west side of the Fab Shop that had a broken safety latch at the throat of the hook; b. An overhead crane at the north side of the Fab Shop that was missing a safety latch at the throat of the hook; c . An overhead crane at the south side of the Fab Shop that had a broken safety latch at the throat of the hook.

This violation occurred on or about September 4, 2013, where the employer did not ensure that fire extinguishers were mounted and were readily accessible to employees in the following instances: a. In the west area of the Fab shop, where a fire extinguisher was not mounted or readily accessible to employees; b. In the Drag Shop, where a fire extinguisher was not mounted or readily accessible at the southwest location of building; c. In the Drag Shop, where a fire extinguisher was not mounted or readily accessible at the southeast location of building; d. In the Fab Shop, where a fire extinguisher was not mounted or readily accessible at the east location of the building.
