29 CFR 1910.179(l)(3)(iii): Repairs or replacements were not provided promptly as needed for safe operation of crane(s): (a) Delta Fabrication & Machine, Inc.: On or before 2/2/23, the employer did not restrict the use of the Cleveland bridge crane (sn: 18160-82) until it had been inspected, approved, or rendered safe to use. The crane's auxiliary hoist had been two-blocked by the host employer and was not taken out of service. Delta Fabrication & Machine, Inc., used the crane's main line despite being aware of the 2-block condition on the auxiliary line. The lower block on the auxiliary line fell due to the rope being damaged by the two-block condition. The lower block struck an employee, breaking his arm and crushing his foot. This condition exposed employees to stuck-by hazards.
