Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     A.)  Combined Systems, Inc.; gas house - On or about 9/10/13 and at times prior thereto, employees were potentially exposed to fires and related smoke inhalation hazards from equipment that did not receive a thorough internal inspection. Equipment, including, but not limited to, the granulator did not receive a thorough internal inspection prior to being putting into operation.  Rust present in the granulator contributed to a fire.

29 CFR 1910.334(b)(2): The repetitive manual reclosing of circuit breakers or reenergizing electric power and lighting circuits through replaced fuses or resetting breakers was not prohibited:    A.)  Combined Systems, Inc.; gas house - On or about 9/10/13, and time prior thereto, the overcurrent devices (e.g. circuit breakers) for the granulator would trip multiple times and be reset by employees.  A qualified person did not examine the electrical system between resets to determine if tripping of the overcurrent device was caused by an overload or a fault condition.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety:    A.)  Combined Systems, Inc.; gas house - On or about 9/10/13,  employees had not received training on the requirements on electrical safety related work practices noted in 29 CFR 1910.334(b)(2).  The overcurrent devices (e.g. circuit breakers) for the granulator would trip multiple times and be reset by employees.  A qualified person did not examine the electrical system between resets to determine if tripping of the overcurrent device was caused by an overload or a fault condition.
