They were not equipped with the hardware fittings necessary if the manufacturer endorses extended uses:    (a) On or about September 16, 2013, a welder was accessing the upper interior wing wall by laying a portable ladder against the fixed ladder in the wing wall, which was located approximately 10 feet up from the bottom.  The two ladders were tied together with sisal rope.  This incorrect usage exposed the welder to a fall hazard.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2):      (a)  On or about September 16, 2013, all the guardrails around the marine vessel were not taut.  The lack of rigidity in the chain created an inadequate guard.  Personnel were exposed to falls of 20 feet.    (b) On or about September 16, 2013, all of the guardrails were composed of chains.  This is not an approved material for guardrails.    (c)  On or about September 16, 2013, on the upper deck, the edge of the deck was open, which exposed welders to a fall hazard.

The driver could come into contact with the electrical wiring and be exposed to electrical shock.  (b)  On or about September 16, 2013, the Hyster Cat Forklift, SN17V268, was found to have a severly damaged rear tire.  Chunks of tire had fallen out and more was ready to fall out.  The damaged tire could cause instability in the forklift and load, which could expose the driver and people in the area to struck by hazards.  (c)  On or about September 16, 2013, the Hyster Cat Forklift, SN17V268, was found to have unlabeled controls.  The labels had worn off and were not replaced.  This could cause the driver to use the wrong control, which could lead to struck by hazards for the driver and nearby coworkers.
